A care manager (certified care support specialist) is a professional who supports individuals requiring care and their families in utilizing care services so that they can live with peace of mind. Their duties include creating care plans (care service plans), coordinating with service providers such as day care and home care services, and providing advice on welfare equipment.
Where are you?
Those certified as requiring Level 1 or higher care will be assigned a care manager from a home care support service provider. If you are unsure which service provider to choose, you can get a referral from your local community comprehensive support center. For those requiring Level 1 or 2 support, the local community comprehensive support center will play a central role in supporting preventative care.
There is no cost to the user for consultations with a care manager or for the creation of a care plan. You can consult with us even if you are at the stage of "not knowing where to start."
It's reassuring to think about clothing preparations early on, such as needing front-opening undershirts or preparing clothes that are easy to change into while in a wheelchair.
